A Film and Video Production Permit is required for any person or company producing a film, television or video production within Forsyth County.

CSS (Customer Self-Service) Portal Instructions
After you create an account, click on the “Apply” tab.
Then click on the “Permits” tab. Scroll through the list to find “Film Permit”.
Click on Film Permit and complete all the necessary fields.
Please be sure to attach all documents through CSS.

- Forsyth County Release Agreement
- Certificate of Liability Insurance
- Site Plan (for each proposed filming location(s) that indicates parking and, if necessary, vehicle access points, temporary signage, road/sidewalk closures, equipment, tent location, etc.)
